What's actually available

Two oral GLP-1 medications are now approved for weight management in adults, and a third oral product — Rybelsus — has long been approved but only for type 2 diabetes, not weight loss.

Do not confuse these with the compounded "GLP-1 tablets" and "oral drops" sold by some telehealth programs. Those are compounded formulations that are not FDA-approved, are not the same products, and have no clinical trial data of their own.

How much weight do they take off?

The trial numbers put both oral options below the strongest injectable results, but comfortably in the range of clinically meaningful weight loss.

In OASIS 4, the pivotal trial for oral semaglutide 25 mg, the estimated mean change in body weight at week 64 was −13.6% for the medication versus −2.2% for placebo. Around 30% of the oral semaglutide group achieved at least 20% weight loss, compared with 3% on placebo.

In ATTAIN-1, the pivotal trial for orforglipron, participants on the highest dose lost an average of 27.3 lbs (12.4%) at 72 weeks, with lower doses producing average reductions of roughly 7.8% and 9.3%.

Why orforglipron is a genuinely different kind of drug

Semaglutide is a peptide. Peptides are fragile — they are degraded by digestion, which is why the injectable form exists and why oral semaglutide needs an absorption enhancer and careful dosing instructions to work at all.

Orforglipron is a small molecule, not a peptide. That is a meaningful manufacturing and logistics difference: small molecules are cheaper to produce at scale, do not require the cold chain that injectable GLP-1s do, and are not subject to the same absorption constraints.

Side effects: same family, same profile

Oral GLP-1s are not a way around GLP-1 side effects. The mechanism is the same, so the side effect profile is the same family: nausea, diarrhea, vomiting, constipation, and abdominal discomfort, clustering around dose increases.

The same serious warnings apply too, including the boxed warning about thyroid C-cell tumors and the contraindication for people with a personal or family history of medullary thyroid carcinoma or MEN 2.
